SUMMARY:Physiological Differences Between Emergency Medicine Residents and
  Novices During Cricothyrotomy: Evidence from Brain, Eye, and Heart Rate M
 easurements

 on-invasive physiological measurements have been used to distinguish level
 s of surgical expertise. However, existing research has primarily concentr
 ated on commonly performed procedures, particularly laparoscopic surgery. 
 This study focused on cricothyrotomy (CCT), a lifesaving emergency airway 
 procedure. Since it is rare, it poses unique challenges for training and m
 aintaining expertise, leading to a reported success rate of only 66% in th
 e US military.\n\nTwelve emergency medicine residents and eighteen novices
  (university students without operative experience) each performed three r
 epetitions of the CCT procedure on a high-fidelity manikin. Multimodal phy
 siological signals, including electroencephalography (EEG), heart rate var
 iability (HRV), and pupil diameter (PD), were recorded. Performance was ev
 aluated using a traditional checklist. T-test was performed on each metric
  to evaluate group differences.\n\nEmergency medicine residents exhibited 
 significantly higher frontal alpha power than novices, indicating more eff
 icient cognitive processing among residents. They also showed a significan
 tly larger increase in pupil diameter, suggesting a higher cognitive workl
 oad. This seemingly contradictory phenomenon was supported by residents' s
 ignificantly higher performance scores and stronger activation in the left
  prefrontal lobe, suggesting their greater engagement. No significant diff
 erence was observed in HRV metrics. These results demonstrate the potentia
 l of physiological measurements to optimize CCT expertise assessment and s
