 ool of Medicine)\n\nDigital health tools like smartwatches and mobile appl
 ications offer promise for supporting lifestyle behavior changes in indivi
 duals living with type 2 diabetes (T2D). Many individuals face barriers in
  adopting digital health tools, as well as interpreting and applying data 
 from these devices. We developed Closing the Loop (CTL), a self-monitoring
  program that integrates multiple streams of data from consumer-grade devi
 ces and apps into a weekly report, supplemented by a comprehensive technic
 al orientation and data-supported health coaching. Twelve individuals livi
 ng with T2D, using a continuous glucose monitor (CGM), and receiving care 
 at a Veterans Administration Medical Center participated in CTL. Participa
 nts used a smartwatch, CGM, a diet-logging mobile application, and complet
 ed daily surveys. They reviewed personalized reports with a health coach t
 o support behavioral changes. CTL demonstrated high feasibility (mean=4.4/
 5), acceptability (mean=4.4/5), and usability (SUS=87.5), with 100% partic
 ipant retention and data completeness ranging from 77–100%. Qualitative fe
 edback highlighted the effectiveness of technical support and the role of 
 shared decision-making in fostering engagement. Findings suggest CTL is a 
 promising, scalable approach to enhancing T2D self-management through inte
 grated digital health tools and personalized coaching. Future iterations w
 ill refine report usability and reduce tracking burden to further support
