  and Ranjit Singh (University at Buffalo)\n\nOlder adults (aged 65 and abo
 ve) are at increased risk of medication-related harm, particularly across 
 transitions of care. These transitions include home to emergency departmen
 t, emergency department to in-patient, in-patient to rehab facility, and i
 n-patient to home. As patients move between settings, handoffs of medicati
 on information and responsibility between multiple actors in complex setti
 ngs may result in medications being inappropriately delivered or various h
 arms not being identified and addressed. In order to identify opportunitie
 s for intervention to minimize these harms, our interdisciplinary team has
  studied this work with three different approaches. Surveys and interviews
  captured patients’ experiences with hospital discharges, workplace observ
 ations captured the cognitive work conducted by hospital staff to ensure m
 edication safety, and data modeling identified contributing factors that p
 redict readmission to the hospital. Considering these three diverse projec
 ts in unison allows for substantive findings regarding potential intervent
 ions to better ensure medication safety across transitions of care.\n\nTra
