Polyp Detection in Colonoscopy Training: A Qualitative Analysis of Resident Challenges and Expert Strategies
DescriptionColorectal cancer (CRC) is a leading cause of mortality, with early detection through colonoscopy being crucial for reducing death rates. However, effective screening depends on a physician’s ability to detect precancerous polyps, with miss rates reaching up to 26%. Simulation-based training (SBT) can improve colonoscopy skills however, physical simulators (PS) which are more realistic than virtual reality simulators, lack integrated feedback, limiting their effectiveness in improving polyp detection (PD). This study identifies the challenges residents face during colonoscopy insertion and polyp detection and explores expert strategies for optimizing PD. Results indicate that residents struggle with colon navigation, scope manipulation, and inspecting folds, while experts utilize techniques such as actively inspecting folds, lumen centering, re-advancing the scope, and slow withdrawal. These findings emphasize the need for targeted feedback on PS to address these challenges. Developing feedback-driven training strategies during simulation-based training using these expert strategies may help reduce polyp miss rates.
