Longitudinal Patient Journey Mapping to Improve Breast Cancer Care: A SEIPS-Based Approach

DescriptionDespite significant efforts over the last 25 years, patient safety and quality improvement efforts remain stagnant (Wears & Sutcliffe, 2019). This may, in part, be due to a continued focus on tasks rather than processes of care - care that is centered on the patient (Walker & Carayon, 2009). Health and healthcare does not occur in a silo, rather, care unfolds across space and time throughout multiple work systems and spanning numerous formal and informal care providers (Walker & Carayon, 2009). Therefore, to truly improve patient safety, we need to expand our focus from tasks to processes of care and to work that spans beyond single care encounters, i.e., the Patient Journey (Carayon et al., 2020). In this study, we describe an approach, SEIPS-based longitudinal patient journey mapping, as a novel method to understand barriers and facilitators experienced by patients across their care journey. This work builds on SEIPS-based process modelling (Wooldridge et al., 2017) and SEIPS-based patient journey mapping (Wust et al., 2022). The objective of this study is to describe the approach and demonstrate its benefit for identifying areas to target for system redesign through a study of breast cancer treatment journeys.
