Interdisciplinary Triangulation of Geriatric Medication Safety Across Transitions of Care
DescriptionOlder adults (aged 65 and above) are at increased risk of medication-related harm, particularly across transitions of care. These transitions include home to emergency department, emergency department to in-patient, in-patient to rehab facility, and in-patient to home. As patients move between settings, handoffs of medication information and responsibility between multiple actors in complex settings may result in medications being inappropriately delivered or various harms not being identified and addressed. In order to identify opportunities for intervention to minimize these harms, our interdisciplinary team has studied this work with three different approaches. Surveys and interviews captured patients’ experiences with hospital discharges, workplace observations captured the cognitive work conducted by hospital staff to ensure medication safety, and data modeling identified contributing factors that predict readmission to the hospital. Considering these three diverse projects in unison allows for substantive findings regarding potential interventions to better ensure medication safety across transitions of care.
